Secure Verification for Registered Patients
Xabat Cardiology Clinic uses one-time verification codes to confirm the identity of registered patients accessing the HeartOS Patient Portal. The verification code is sent by SMS to the mobile number recorded in the patient’s clinic registration details.
The patient must enter the correct verification code before accessing patient-specific services, including the secure submission of home blood pressure readings for clinical monitoring.
How the Verification Code Is Used
- Confirm that the person attempting to access the portal is the registered patient.
- Protect patient information from unauthorised access.
- Securely authenticate the patient before access to HeartOS patient services.
- Permit an authorised patient to submit home blood pressure readings requested by the clinic.
- Protect other patient-specific services that may later be made available through HeartOS.
The verification code is used only for authentication and transactional security. It is not used for advertising, marketing or promotional messages.
Patients must not share their verification code with another person. Xabat Cardiology Clinic staff will never call, message or ask a patient to disclose the code.
A patient who receives a verification code without attempting to access HeartOS should not share the code and should contact the clinic.
Home Blood Pressure Monitoring
Authorised patients may use the HeartOS Patient Portal to submit home blood pressure readings requested by their clinician.
Information submitted may include systolic blood pressure, diastolic blood pressure, pulse rate, date and time of measurement, relevant symptoms and patient notes.
Submitted readings may be reviewed by authorised clinical staff of Xabat Cardiology Clinic.
Submitting a blood pressure reading through HeartOS does not replace emergency medical assessment.
Patients experiencing chest pain, severe difficulty breathing, fainting, sudden weakness, confusion or other urgent symptoms should seek immediate medical care.
Privacy and Patient Information
Verification codes and patient information are used only for patient authentication, patient care, clinical monitoring and the secure operation of HeartOS.
Access to patient information is restricted to the patient and authorised members of the Xabat Cardiology Clinic team.
Xabat Cardiology Clinic does not use verification codes to deliver promotional messages.
